What Is Direct Primary Care and How Does It Work?
Direct Primary Care (DPC) is built around you , not the business of healthcare. With a DPC membership model, you pay a low flat monthly fee instead of billing insurance. The fee covers unlimited visits, longer appointments, same day or next day appointments, clinician’s phone number, telehealth, home visits, and more.
Gone are the rushed 15 minute visits and long wait times for an appointment. You will receive the time, attention and expertise both during and between visits. As an internal medicine practice, we care for adults with a wide range of health needs from routine preventive care and acute illnesses to the management of complex chronic conditions. We also have extensive experience caring for older adults, providing thoughtful, comprehensive geriatric care that adapts to changing health needs over time.

Mountain Internal Medicine is co-owned by best friends Dr. Sarah Faaborg, MD, and Marnie DiChiaro, AGNP-C, and has served adults in downtown Missoula since 2022. We keep our clinic small on purpose, so you see the same care team every visit.

We are currently accepting new DPC patients, but unfortunately our panels are full for Medicare or Medicaid insured patients.
Membership is $100 per month for one adult, covering unlimited visits and telehealth access. If two or more family members in the same household join together, the rate drops to $85 per person per month. Employer groups of ten or more also qualify for a reduced rate of $75 per member per month.
Yes to both. Geriatric care is one of Marnie DiChiaro's specialties, and she offers home visits for patients who have a hard time getting to the clinic. Old fashioned medicine is becoming more difficult to find in Missoula.You don't need a complex condition to be seen here, either; we support everything from routine adult physicals to more complicated care with multiple or challenging diagnosis.
Direct Primary Care membership runs outside of traditional insurance billing. One monthly fee, unlimited care, without co-pays, deductibles, or surprise bills. Many members still keep insurance for hospital visits, specialists, prescriptions, and other non-covered DPC benefits.
No. We handle scheduling by phone to ensure you have the proper time required for your concerns and health needs.
Yes. Members can request telehealth primary care visits for many follow-ups and quick concerns.

Many HSA plans allow you to use funds toward direct primary care membership fees, but eligibility depends on your specific plan. Check with your HSA administrator before enrolling so you know exactly how your plan treats DPC payments.
Beyond primary care, we offer PRP therapy for joint, soft tissue, and hair restoration, plus our PHAT medical weight loss program with clinician-guided GLP-1 and GLP-1/GIP options. We also run point-of-care testing, like A1C, strep, flu, and RSV checks, right here in the office for quick diagnosis without additional cost.
